COURSE OBJECTIVE Upon the completion of this module you will be able to conduct an incident investigation, identify causes and remedial actions using the root cause analysis techniques and prepare an investigation report. LEARNING OUTCOMES The participant will be able to:
SYNOPSIS Incident investigation techniques involve a methodical examination of an undesired event that did/could result in harm/damage to people/property. Investigation activities are directed towards defining the facts and circumstances related to an event, determining the causes and developing remedial actions to control the risks. This learning module deals with the techniques and tools required to perform an incident investigation and use the root cause analysis technique. |
