COURSE OBJECTIVE

  • Know and apply the management process for a “hazard”, preventing its “loss of control”, as well as regaining control and mitigating its severity - “HOW”

  • Understand “WHAT” it takes for an accident to happen.

  • Realize the “organizational weaknesses” causing its occurrence – “WHY”.

  • Appreciate the relationship between the results at the front end of the business.

  • Able to identify hazards and estimate risks in relation to accidents.

  • Able to identify the root-cause of the accidents.

  • Able to recognize the control measures to avoid accidents from occurring.

SYNOPSIS

Tripod Beta Accident Investigation course (TBAI) is to enlighten the participant an understanding of the mechanism – “what”, “how” and “why”, leading to an accident and linking it to the hazard management process – “bowtie”.

It is a very beneficial course to train workers in conducting proper accident investigation in the offshore or offshore environment. These three-day course is useful for manager level in the oil and gas industry to understand the methodology of accident investigation.
